COJO Flatbush is hiring a full-time Case Manager to support students enrolled in our Adult Education
Programs, which include GED-Prep and Computer Classes. The Case Manager plays a key role in guiding
clients through challenges, developing actionable plans, and connecting them with essential resources
to ensure their long-term success.
Key Responsibilities:
- Provide comprehensive case management services which includes but not limited to, intake assessments, setting goals, and developing action plans for COJO Students.
- Offer education and career counseling to assist students in identifying pathways to reach personal, educational, and professional goals.
- Facilitate coordination with external service providers, government agencies, and community organizations to build a strong support system for participants.
- Maintain and update participant records, ensuring accurate documentation in DYCD Connect to track interactions and outcomes.
- Conduct regular follow-up meetings with participants to monitor progress and adjust goals as needed.
- Lead workshops and group sessions focused on education, career development, and family support resources.
- Work collaboratively with the Adult Education team to provide holistic, integrated support.
- Stay updated on local resources, policies, and best practices in family development and social services.
- Candidates must meet one of the following criteria:
- Bachelor’s degree in social work or a related field, with experience in City systems (public assistance, child welfare, education, and housing); OR
- Associate’s degree, completion of Family Development Training (or other DYCD-recognized credentials), and experience in City systems; OR
- At least 3 years of proven experience in case management or counseling, with knowledge of City systems.
Annual Salary: $40,000 - $45,000 based on experience + Benefits
