The Brooklyn Public Library Adult Learning Program provides student centered adult literacy services throughout Brooklyn communities. We are seeking a part-time floater Case Manager to support the needs of HSE participants at various programming sites. The case manager will help ensure that our program participants are knowledgeable of free programs/ resources that will help alleviate any barriers to completing their educational goals.

Responsibilities
  • Conducts a comprehensive literacy and educational/vocational assessment to develop a plan of service to meet participant’s needs (approximately 50 clients per year).
  • Identifies post-secondary transition programs and assists out-of-school youth and adults to earn their high school diploma, improve their English skills, and to improve their reading and writing skills and succeed in post-secondary education, apprenticeship programs, or training.
  • Communicate effectively and collaborate with key staff and instructors to ascertain barriers that specific participants may be experiencing that prevent them from meeting their educational needs.
  • Collaborates with and refers participants to various community partners for wrap-around services, including health literacy, financial literacy, and more.
  • Researches and assists participants in acquiring related supportive services such as computer literacy, transportation, public assistance and child care.
  • Facilitates and/or coordinates various workshops/information sessions.
  • Attend community partner meetings and attend bi-monthly case management team meetings.
  • Maintains case notes by documenting client’s goals, outcomes, and goals using the ASISTS database.
  • Monitors participant's progress and follows up frequently.
  • Performs other tasks deemed necessary to assist in achieving service and programmatic goals.
Qualifications
  • Some college required, Bachelor’s degree preferred in education or a related field; relevant experience in adult education (ABE, ESOL, & HSE)
  • Strong awareness of the barriers facing adult learners looking to integrate workforce, technology, and academic environment.
  • Relevant experience in counseling individuals and/or facilitating informational sessions
  • Must have a good knowledge of the available educational, training, and employment resources.
  • Good knowledge of labor market trends and requirements and the skills, abilities, and experiences required in various occupations.
  • Initiative and resourcefulness; tact and courtesy; Good judgement and problem solving ability
  • Proficient in MS Word, Excel, Google Drive, Outlook, Social Media
  • Able to travel to various program sites throughout the week and work alternate Saturdays, evenings, and day shifts

The LZ case manager will help ensure that program participants are knowledgeable of free programs/ resources that will help alleviate any barriers to completing their educational goals. They will work collaboratively with the WIOA HSE admin team and instructors to ensure participants’ needs are being met. This role will work closely and under the guidance of the Literacy Zone coordinator onsite.

Responsibilities
  • Conducts a comprehensive and educational/vocational assessment to develop a plan of service to meet participants’ needs (approximately 100 clients per year). Provide intensive one-on-one and/or small group services to participants and their families including counseling and advising participants on their career, academic, and social service needs. The goal is to link them with resources that would move them out of poverty.
  • Able to plan events and coordinate community events based on the Literacy Zone target (goal is to meet and/or exceed program targets)
  • Identifies post-secondary transition programs and assists foreign-born participants to become familiar with the American workforce, post-secondary youth and adults to earn their high school diploma, improve their English skills, and improve their reading and writing skills and succeed in post-secondary education, apprenticeship programs, or training.

JOB DESCRIPTION
Summary/ Objective
East Side House Settlement (ESH) is a community resource in the South Bronx. We believe education is the key that enables all people to create economic and civic opportunities for themselves, their families, and the communities.  We seek a dynamic, hard-working, and creative team-player who shares our passion for providing exceptional services to children and families.

Under the supervision of the Senior Director of Community Schools, the Case Manager will be key in the Community School and Workforce staff inter-disciplinary team whose purpose is to provide comprehensive educational and social emotional supports that enable participants to meet their learning, educational and employment goals. A primary focus for the Case Manager is to support our ALE participants throughout their process in the Test of Adult Basic Education (TABE) and Workforce services. The Case Manager will communicate effectively with the Senior Director of Community Schools and Workforce team to coordinate student intake and interviews, tutoring and all other wrap around services needed for participants, who are enrolled in various classes, to succeed.

Essential Functions
  • Duties and responsibilities include, but are not limited to:
  • Manage a caseload of approximately 50-100 participants per year; divided by cohorts.
  • Maintain ongoing contact with participants on caseload at all phases of the ALE program to track progress; meet individually at least twice per course with every adult participant.
  • Conduct daily attendance outreach and refer participants to support services as needed.
  • Maintain consistent communication with the Instructors and Career Development Specialists to ensure participants career goals are met.
  • Conduct one-on-one and small group counseling sessions with participants that support their academic, career and personal goals.
  • Work as part of an interdisciplinary team with Instructors, Career Development Specialists, and Education and Employment Services Managers in providing onsite/co-location services.
  • Build and maintain Google shared database with case load of participants to track participant progress through career and education goal plans.
  • Participate in relevant departmental meetings, training, case conferences, workshops and other on/off site events.
  • Assist the program management staff in recruiting, interviewing, and maintaining participant records for each cohort of ALE.
  • Consult with teachers and provide feedback to participants on academic performance using a biweekly assessment.
  • Refer participants to academic and support services as needed.
  • Attend administrative meetings, program trainings, and staff development.
  • Educating participants and families about post-secondary options, colleges, the college admissions process, trends, procedures, and testing; advising participants and families as they go through the process and helping participants and families aspire realistically and choose wisely.

OBT is looking for a youth-focused, dedicated Case Manager to join our Advance and Earn Program team.  Reporting to the Social Service Supervisor, the Case Manager will be responsible for providing counseling and case management services to a minimum of 19 participants at all stages of the Young Professional’s Institute (YPI) program in Advance and Earn. 
 
In partnership with the Department of Youth and Community Development, OBT launched the Advance and Earn Program in 2020. The program, a unit of OBT’s Young Professional Institute, helps opportunity youth,  those young people between the ages of 17 and 24 who are neither enrolled in school nor participating in the job market, further their career prospects through comprehensive High School Equivalency (HSE) test preparation, employer-recognized trainings, credentials and certifications, and paid internships.  There are 3 main components of the program: contextualized academic programming, comprehensive HSE and work-based learning, and sector-focused advanced training in Digital Marketing. 
   
Essential Job Functions and Responsibilities: 
  • Provide strength-based case management and counseling to young adults at all phases of the Advance and Earn model. 
  • Maintain case tracking, case conferencing and group supervision models to assure that young adults and staff are holistically supported. 
  • Participate in clinical supervision.  
  • Provide case management and crisis management as required. 
  • Attend relevant professional development and provide referrals for external training for staff. 
  • Develop and facilitate essential skills workshops. 
  • Monitor the flow of young adults throughout program components. 
  • Enter and track data in all relevant systems. 
  • Input case notes and participant assessments. 
  • Participate in all team meetings, required trainings, and case conferences. 
  • Work cohesively with an interdisciplinary team to provide multi-tiered interventions. 
  • Monitor and track student attendance.
