Part-Time Case Manager Positions (Bilingual Spanish Preferred)
Adult Basic Skills (ABS) Program at LaGuardia Community College
LaGuardia Community College is seeking two part-time case managers for its ABS High School Equivalency preparation program for 19 hours per week. This involves some day and evening hours and an occasional Saturday.
​
Case Manager must be comfortable with addressing barriers that will prevent students from pursuing their goal. Case Manager must work closely with teachers and administrative staff to provide the necessary support for the students in our program. learner-centered, participatory lessons and a project-based approach.
We are seeking a qualified case manager who has experience working with adult students. BA/BS degree required in social work or related field, plus a minimum of 2-3 years of experience working with adults as a case manager. Bilingual (English/Spanish) preferred.

Primary Responsibilities include:
  • Providing high quality support services to students we serve in the program
  • Interviewing prospective students to discuss possible barriers
  • Identifying and addressing issues as childcare, housing, family illness, work schedule, time management, finance, and other obstacles preventing students to complete their course
  • Communicating with students regarding absenteeism
  • Motivating students to persist and succeed in the program
  • Developing and presenting workshops on how to study, test anxiety, time management, financial literacy, etc.
  • Assisting students to complete GED applications
  • Conferencing students individually on their goals and next steps
  • Collaborating with the Career Development Center to assist students in applying to college
  • Referrals to appropriate training programs, employment opportunities and/or community agencies
  • Participating in professional development and staff meetings
  • Other related tasks, as necessary
  • Part-time positions 19 hours per week (flexible hours) through June 2023 (with the possibility of extending past June 2023)

OBT is looking for a youth-focused, dedicated Case Manager to join our Advance and Earn Program team.  Reporting to the Social Service Supervisor, the Case Manager will be responsible for providing counseling and case management services to a minimum of 19 participants at all stages of the Young Professional’s Institute (YPI) program in Advance and Earn. 
 
In partnership with the Department of Youth and Community Development, OBT launched the Advance and Earn Program in 2020. The program, a unit of OBT’s Young Professional Institute, helps opportunity youth,  those young people between the ages of 17 and 24 who are neither enrolled in school nor participating in the job market, further their career prospects through comprehensive High School Equivalency (HSE) test preparation, employer-recognized trainings, credentials and certifications, and paid internships.  There are 3 main components of the program: contextualized academic programming, comprehensive HSE and work-based learning, and sector-focused advanced training in Digital Marketing. 
   
Essential Job Functions and Responsibilities: 
  • Provide strength-based case management and counseling to young adults at all phases of the Advance and Earn model. 
  • Maintain case tracking, case conferencing and group supervision models to assure that young adults and staff are holistically supported. 
  • Participate in clinical supervision.  
  • Provide case management and crisis management as required. 
  • Attend relevant professional development and provide referrals for external training for staff. 
  • Develop and facilitate essential skills workshops. 
  • Monitor the flow of young adults throughout program components. 
  • Enter and track data in all relevant systems. 
  • Input case notes and participant assessments. 
  • Participate in all team meetings, required trainings, and case conferences. 
  • Work cohesively with an interdisciplinary team to provide multi-tiered interventions. 
  • Monitor and track student attendance. 
  • Develop and facilitate retention events for students. 
  • Engage participant stakeholders, such as families and community partners to ensure best outcomes for youth. 
  • Monitor and facilitate intakes and ongoing student assessments throughout program. 
  • Participate in internal trainings on Motivational Interviewing (MI), and employ MI strategies as a primary modality of practice.   
  • Other assignments as needed in support of supervisor and managers.

Position: Evening Case Manager

Union settlement Adult Education is seeking a Case Manager to provide in person support that reports to the Adult Education Director. The candidate needs to be a hardworking individual with the ability to multi-task effectively. This person is responsible for attendance tracking, monitoring and data entry into Adult Education Funder databases as needed. This is a full-time position.

DUTIES INCLUDE:
  • Processing intake application with proper documentation.
  • Schedule and confirm appointments for clients.
  • Make copies for staff and scan necessary documents for files.
  • File, retrieve and update documents and reports.
  • Maintain and log daily sign-in sheets for students.
  • Provide timely data entry in DYCD Connect and other Adult Education Funder databases which also includes enrolling students and assisting with programming.
  • Assists participants in obtaining vital social services within the neighborhood.
  • Assessing participants needs for community resources
  • Provide referral services to other Union Settlement programs when necessary, as well as to other external resources.
  • Create monthly program reports detailing all services provided for participants.
  • Attend community events to promote Career Training program alongside other Union Settlement programs
  • Maintain case records and program documentation in accordance with agency and program guidelines.
  • Conduct workshops that provide necessary life skills for participants.
  • Work with participants to assess strengths, needs and resources; develop an individualized strategy to meet short- and long-term goals; and follow up to determine whether goals have been met and/or needs have changed.
  • Develop and maintain resource information database for future use.
  • Conduct periodic assessments of enrolled participants to determine progress.
  • Develop and manage an Individualized Service Plan coordinating with Participants and other community resources and agencies.
  • Teach problem solving and appropriate coping techniques to enhance participant life skills function.
  • Develop and maintain resource information.
  • Follow-up on referrals you make on behalf of a participants and respond to all referrals made to the program.
  • Maintain linkages to ensure documentation and verification of participant outcomes.
  • Participate in training opportunities as assigned.
  • Maintain and ensure HIPAA confidentiality of records (daily)

PHYSICAL REQUIREMENTS 
  • Regularly required to talk, hear, walk, stand, & sit. 
  • Able to climb stairs if necessary. 
  • Able to operate a computer keyboard, mouse & office equipment. 
  • Ability to read printed materials and computer screens.

QUALIFICATIONS: 
  • Associates degree in social services or related field with two years’ experience in a social service setting or in providing direct services to individuals or families or Bachelor degree in social work or related field.
  • Must be bilingual (English and Spanish.)
  • Ability to work both independently and as a team with an equal amount of focus and enthusiasm. 
  • High degree of discretion and respect for confidentiality
  • Good verbal and written command of the English Language.

The Lehman College Adult Learning Center (ALC) is a teacher-led direct-service project of the Institute for Literacy Studies (ILS). Our shared mission—advancing urban education—has deep significance for the schools, colleges, and community-based settings in our home borough of the Bronx. Four interrelated core values inspire and infuse our work: commitments to human capacity, social justice, the power of democratic communities, and transformative work for all persons. 

We provide thoughtful, innovative instruction, individualized counseling, and a nurturing environment for learners aged 16 and older. Students enter our program with the hope that the ALC’s key programs comprising free classes for Adult Basic Education (ABE), High School Equivalency (HSE), and English for Speakers of Other Languages (ESOL), including career and college readiness, can provide them with a meaningful pathway toward qualitatively better lives and economic self-sufficiency. GENERAL DESCRIPTION OF DUTIES:

Reporting to the Director of the ALC, the Case Manager will perform the following duties:
  • Act as a confidential case manager for adult student issues, including assessing academic and personal needs and assisting students who may be in crisis or off-track with ALC program requirements.
  • Keep an updated case management log accessible to the Director.
  • Refer students to support services, academic and career advising, CUNY, and external resources to assist with issues, including legal services, academics, crisis support, wellness, and emotional support. Develop relationships and collaborates with local agencies and community organizations to provide students with additional avenues to access resources and programs.
  • Create and share information and resources with ALC students, staff and faculty, and the Lehman community by hosting workshops and events.
  • Arrange and assist with special events, ceremonies, and conferences.

Attributes of Lehman’s Adult Learning Center staff members are:
  • Experienced advising immigrant and underrepresented students with a strong understanding of student support and academic services in education.
  • Informed about trends and issues facing immigrant, low-income and first-generation college, underrepresented, and disadvantaged students in education.
  • Experienced in analyzing problems and identifying solutions as a team.
  • Able to work effectively with diverse constituencies, including urban students, faculty, and administrators.

Center for Family Life in Sunset Park is seeking to hire a full time Bilingual Case Manager
 
The Center for Family Life is a neighborhood based social service agency offering a comprehensive range of services to children and adults while contributing to community building. The services include family counseling, adult and youth employment services, emergency food and extensive social, educational, and recreational programming at various local schools.

The Adult Employment Program is an open admissions job readiness program that assists unemployed and underemployed participants with their search for work. The services include job readiness preparation, assessment and referrals according to participants' needs. The Adult Employment Program is currently looking for a Case Manager to help unemployed job seekers with their search for work and to provide case management services to immigrant families.
​
The Case Manager will be a full-time (35 hours/week) staff member of Center for Family Life. The Case Manager will be responsible for providing culturally competent, strength-based case management services to immigrant families whose primary language is Spanish.
 
Responsibilities include but are not limited to:
  • Conducting intake interviews to identify families who qualify for the program
  • Facilitating Orientation and other workshops for new participants, and providing detailed information about the Program, all other services available at CFL and at community partners and other City-funded programs that support immigrant families.
  • Collaborating with families to develop a Comprehensive Needs Assessment and a Family Services Plan (FSP)
  • Connecting family members to services identified in the FSP, including scheduling appointments, maintaining communication with service providers, and following up on appointments; Accompanying clients to appointments as needed
  • Meeting with each family at least bi-weekly; Reassessing the needs of family members and service effectiveness to inform changes to the FSP
  • Coaching family members to develop self-advocacy skills and become self-sufficient as they navigate the complex systems that affect their lives and independently access the services and benefits they need
  • Monitoring the implementation of the FSP and participants’ progress toward the short- and long-term goals identified in the FSP
  • Maintaining timely case records and documentation requirements, including recording case information, progress notes, and all required documents in a Department of Youth and Community Development online database system
  • Participating in individual supervision with Program Director and in weekly group supervision/professional development sessions for CFL’s casework team.
 
Qualifications:
  • Bachelor’s degree in Psychology, Sociology or a related subject area
  • Relevant case management experience working with children and families in low-income, immigrant communities
  • Fully Bilingual in English and Spanish
  • Strong time management skills and group work skills

Educational Case Manager
The Adult Learning Center (ALC) at NYC College of Technology seeks a part-time Educational Case Manager (ECM) for our Integrated Literacy and Civics Education (IELCE) funded program. The ALC collaborates with workforce development partners to provide advanced English learners free healthcare training which includes contextualized English instruction.
Primary responsibilities:
  • Advise and support advanced level English language learners enrolled in Medical Assisting and Direct Support Professional trainings, through in-class workshops and one-on-one sessions. Counsel participants on career prospects, academic goals, and personal concerns, while ensuring confidentiality
  • Identify and establish joint referral relationships with relevant social service agencies, community-based organizations, and with Workforce1 centers
  • Assess student needs and make appropriate referrals to social services and other agencies
  • Provide crisis intervention when needed
  • Assist students with mapping their career plans, including related academic and personal goals
  • Track student progress, attendance and satisfaction, while completing required data entry tasks
  • Work closely with instructors and administrative staff to meet program objectives; this includes the monitoring and documentation of progress and outcomes for both current and former program participants
  • Provide support during both the application process and the orientation of new students
  • Attend 14 hours of RAEN Professional Development
  • Ad hoc duties as assigned
Requirements:
  • Bachelor’s degree in a social work, counseling, or a related field. Master’s degree preferred
  • Relevant experience in counseling individuals
  • Strong awareness of the barriers facing immigrants looking to integrate into the US workforce and academic environment
